ANNUAL PCP KICKOFF MEETING 2012
We have
scheduled the following physician meetings to share our 2012 P4P criterias
and our collaborative efforts in maximizing your performance scores. In
addition, we would like to share with you the different initiatives the IPA
has in place for 2012 in our commitment to quality care for our patients and
to exceptional service to your practice; as well as provide you with the
latest updates on the IPA’s EHR Program, 5 Stars, Access Standards, and ACO
updates. Allow us to assist you as you take a participatory role in making
2012 a successful P4P year!

2012 MEASURE UPDATES
BREAST CANCER SCREENING (BCS)
Description:
The percentage of
women 40-74 years of age who had mammogram to screen for breast
cancer within the two-year period.
Exclusion: Women who
had a bilateral mastectomy.
Alamitos IPA, Lakewood IPA, and St. Mary IPA in
collaboration with Preferred Diagnostic Imaging (PDI) is now holding
a “Screening Day” every 3rd Saturday of each month at PDI
for their convenience, beginning January thru December 2012.
Upon completion of the screening, patient will receive a free gift (a $10
value) and a raffle entry to our monthly grand prize drawing. Any other
appointment dates made for that month will be an automatic entry to the
monthly drawing. This is an appreciation for taking care of your health.
EVIDENCE-BASED CERVICAL
CANCER SCREENING (ECS)
Description: Women 21
years of age and older who were appropriately screened for cervical cancer
in accordance with evidence-based standards within the three-year period:

24-65 Years With No Hysterectomy |
24-65 Years With Hysterectomy |
67 Years and Older |

Perform only one pap smear
within three years from the measurement year |
No pap smear is performed within three years from the
measurement year |
No pap smear is performed within the measurement year |

*Guidelines are based on a normal screening result. |
Exclusion: Women who
have had a diagnosis of dysplasia, HPV, or an abnormal Pap test in the past
five years; a history of cervical cancer, HIV or DES exposure.
Alamitos IPA, Lakewood IPA, and St. Mary IPA
invite
your patients to “Screen To Win” during the entire month of
January. Completion of the screening during this month will receive a $5
gift card and a raffle entry to a grand prize of a $100 gift card of
their choice that will take place on January 31, 2012. This is
our appreciation for taking care of your health.
